Surgeons can work with a wide range of patterns and conditions as long as the hair at the back and sides of your scalp remains genuinely stable and unaffected by miniaturisation. If it doesn’t, the whole premise of a transplant β€” that you’re moving permanent, DHT-resistant hair to thinning areas β€” breaks down.

The Miniaturisation Problem β€” Explained Simply

Miniaturisation is the process by which DHT (dihydrotestosterone) gradually weakens hair follicles. Each affected follicle produces a progressively thinner, shorter hair until eventually it stops producing hair entirely.

In standard male pattern baldness, this process is concentrated in specific zones β€” the frontal hairline and crown β€” while the occipital region (the back of the head) remains largely immune because those follicles are genetically insensitive to DHT.

Finasteride

Finasteride works by inhibiting the conversion of testosterone to DHT, reducing the hormonal signal that causes follicle miniaturisation. For many men, it significantly slows or halts ongoing loss. For diffuse thinners with AGA, it can sometimes produce noticeable regrowth in partially miniaturised follicles.
